45-minute consult on products and services offered, height, weight, and BMI with a customized medically supervised plan. To start injections or medications you must have fasting labs within the last 60 days, you may bring your labs to your visit or we will perform for a reduced fee of $95, you must fast for 8-10 hours if labs will be drawn at the visit. If you are not fasting the day of your visit, we will schedule an appointment for weight management fasting labs. Labs included: Fasting Lipid Profile, Fasting Insulin, CMP, CBCD, TSH, Vitamin D, Vitamin B12.
Gold Weight Loss Membership
$159/month
The GOLD Weight Loss membership includes a weekly weigh in visit with the nurse or medical assistant and a supplement/lipotropic injection with added vitamins each week. In addition, you will meet with a board-certified nurse practitioner every 4-8 weeks (telehealth provider visits are available for those living out of town). The InBody scan is included with your GOLD membership every 12 weeks. Plus, 10% off vitamins and supplements in office. Clients with a history of kidney stones are not a candidate for lipotropic injections. Semaglutide and Tirzepatide are available for those who qualify at an additional charge.

SELECT Weight Loss Select Membership
$79/month
The SELECT Weight Loss membership includes a weekly weigh in visit with the nurse or medical assistant (supplement/lipotropic injections are not included with this membership type, but you may purchase an injection at your visit). In addition, you will meet with a board-certified nurse practitioner every 4-8 weeks (telehealth provider visits are available for those living out of town). The InBody scan is included with your SELECT membership every 12 weeks. Plus, 10% off vitamins and supplements in office. Maintenance Weight Loss Membership
$39/month
The Maintenance Weight Loss membership includes a once-a-month weigh in visit with the nurse or medical assistant (supplement/lipotropic injections are not included with this membership type, but you may purchase an injection at your visit). In addition, you will meet with a board-certified nurse practitioner every 12 weeks to discuss how the maintenance phase of your weight loss is progressing (telehealth provider visits are available for those living out of town). Maintenance is only an option for those with a paid weight loss membership for a minimum of 6 months, who have maintained their weight loss for a minimum of 3 months. This is a great option for those who have lost weight and would like the accountability and continued support from Select Health. The InBody scan is included with your Maintenance membership every 12 weeks. Plus, 10% off vitamins and supplements in office. Clients with a history of kidney stones are not a candidate for lipotropic injections. Semaglutide and Tirzepatide are available for those who qualify at an additional charge.
