What is the membership cost?
$29/month for an adult (18-29 years)
$39/month for an adult (30-39 years)
$49/month for an adult (40-49 years)
$59/month for an adult (50-69 years)
$69/month for an adult (70+ years)
$69 Wellness/Primary Care Consultation
These charges are for primary care services as included in our “What’s Included” list above.
Membership is NOT insurance, and patients are strongly recommended to have insurance to cover specialty costs and catastrophic fees for emergency room and hospitalizations.
While we do realize that there are times when non-members need an acute care visit or urgent care, the Select Health concept is based on continuity of care and provider-patient relationships. We will see you for a non-member visit for $65 for an uncomplicated new patient visit and $60 for an established patient visit. If you would like an appointment for an acute care visit or wellness exam, we do not accept walk-in visits but will try to work you in for a same-day appointment or next day appointment. If an out of town family member of one of our current DPC members would need care for and acute illness, we would be happy to accommodate them for a one-time fee of $65.
